Evergreen Wellcare offers primary care, dental care, weight management, fitness coaching, aesthetics, and holistic wellness services. Results and recommendations are provided for informational purposes and do not replace professional medical advice from a licensed physician.

2. Appointments & cancellations

Appointments may be rescheduled or cancelled up to 12 hours in advance without charge. Same-day cancellations may be subject to a partial visit fee.

3. Accuracy of information

You agree to provide accurate personal and health information at the time of booking. Evergreen Wellcare is not responsible for delays or errors caused by incomplete or inaccurate information provided by the patient.

4. Payment & pricing

All prices are listed in US dollars and are subject to change with prior notice. Payment is required at the time of booking unless otherwise agreed in writing or covered by insurance.

5. Limitation of liability

Evergreen Wellcare partners only with board-certified clinicians and accredited laboratories; however, we are not liable for indirect or consequential damages arising from the use of our services beyond the value of the service purchased.
